For some reason, shortly after I got clean URL's to work some of my icons disappeared and instead showed some garbage like the screenshot below.

What I did first was clear the cache in my browser and even tried a different browser on a different machine... no luck

Next I tried clearing the cache on Open Atrium... no luck

Within the theme I clicked on Scan for breakpoints... no luck

Then I switched from the default theme to something else as default then back to the original theme... FIXED
